Use Form 3911 to ask IRS to Trace or Replace a Refund Check.
Occasionally, a refund gets lost, stolen, or perhaps transmitted to an
incorrect bank account.
A taxpayer can speed up the process of receiving a replacement check by
submitting IRS Form 3911, Taxpayer Statement Regarding Refund. This form
requests the IRS to trace the refund and to issue a replacement check, if
applicable. It is divided into three sections and should be completed, signed,
and mailed to the IRS center that processed the tax return.
Form 3911 can be
obtained from the IRS or viewed, printed, or downloaded from this page.
